Streszczenie

Metoda otwartego brzucha (OA) po raz pierwszy zastosowana w ramach chirurgii kontroli uszkodzeń w warunkach urazowych, znajduje zastosowanie jako strategia leczenia ciężkich urazów, zespołów ciasnoty przedziału powięziowego, ostrych zapaleń trzustki, nagłych stanów naczyniowych, sepsy brzusznej czy powikłań leczenia chirurgicznego u pacjentek chorych na raka jajnika. Istotnym elementem koncepcji OA jest laparostomia, czyli zabieg, podczas którego jama brzuszna jest celowo pozostawiona otwarta, a zawartość trzewno-otrzewnowa jest tymczasowo chroniona różnego rodzaju metodami zabezpieczającymi. Metoda ta umożliwia sekwencyjne usuwanie zakażonych i martwiczych tkanek oraz rekonstrukcję narządów wewnątrzbrzusznych przy zachowanej stabilizacji hemodynamicznej, co przyczynia się do poprawy wskaźników przeżywalności pacjentów. W niniejszym przeglądzie omówiono metody służące do wykonywania zabiegów tymczasowego zamknięcia brzucha.
