Abstrakt:
Przepuklina przystomijna (Parastomal Hernia - PSH) jest to przepuklina pooperacyjna znajdująca się w stomii lub bezpośrednio do niej przylegająca. Szacowana częstość występowania PSH wynosi 30% po roku od wyłonienia stomii, a przy dłuższej obserwacji wynosi nawet 50%. Leczenie operacyjne jest wymagane wśród około 30% pacjentów z PSH. Najczęściej wykorzystywaną metodą leczenia chirurgicznego PSH jest operacja z wykorzystaniem siatki techniką Sugarbakera (Sugarbaker-SB) lub techniką „dziurki od klucza” (Keyhole-KH). Dobór odpowiedniej techniki w operacyjnym leczeniu PSH ma bardzo duże znaczenie w kwestii odsetka nawrotów PSH. W porównaniu metod SB i KH zaobserwowano, że wśród pacjentów operowanych techniką SB występuje znacznie mniejszy odsetek nawrotów PSH niż w grupie pacjentów operowanych metodą KH. Pod względem rezultatów krótkoterminowych techniki SB i KH wykazywały podobne wyniki. Zmodyfikowane warianty SB i KH wykazywały się lepszymi wynikami w porównaniu do swoich standardowych odpowiedników. Porównywane grupy cechowały się znaczną heterogenność pacjentów, co ogranicza możliwości uogólnienia przedstawianych rezultatów. Brakuje danych odnośnie długofalowego wpływu doboru techniki leczenia operacyjnego PSH, a częstością nawrotów PSH, co wymaga prowadzenia dalszych badań.

Abstract:
Parastomal hernia (PSH) is a postoperative hernia that occurs at or in direct proximity to a stoma. The estimated incidence of PSH is approximately 30% within the first year following stoma creation, increasing to as much as 50% with longer follow-up periods. Surgical intervention is required in approximately 30% of patients diagnosed with PSH. The most commonly employed surgical techniques for the treatment of PSH involve the use of mesh, typically through the Sugarbaker (SB) technique or the Keyhole (KH) technique. The selection of an appropriate technique in the surgical treatment of PSH is crucial in terms of the PSH recurrence rate. Comparative studies have demonstrated that the recurrence rate of PSH is significantly lower among patients treated with the SB technique compared to those who underwent repair using the KH technique. In terms of short-term outcomes, both the SB and KH techniques yield comparable results. However, modified versions of the SB and KH techniques have shown superior outcomes relative to their standard counterparts. The analyzed patient groups exhibited considerable heterogeneity, which limits the generalizability of the reported findings. Furthermore, there is a lack of data regarding the long-term impact of surgical technique selection on PSH recurrence rates, highlighting the need for further research.
